There is a web application that you complete. However, you must first become a member of the site by creating a user id; this ensures that the database stays "clean" and free from (spam)bot submissions.
Please be as clear as you can be on your purpose and who you are meant to serve and make sure to include it in the description of the services you offer. To see an example of what I mean look at my essence, blessing, and mission statements on my profile page. After you input your information, I will need to approve the request to be added to the database.

By submitting your information to the database you agree to allow my referrals to provide me with ratings in a number of different areas of their experience in using your services, which will be aggregated into your table entry. I will do my best to include only legitimate feedback. See this FAQ for more information.
If you don't wish to become part of the database, you may simply post your information in the Service Provider:Introduce Yourself section of the Forums.
As one of my service providers I ask that you pay me a referral fee. I suggest 20% of the ongoing revenue you generate from my referral. However, you may pay whatever amount and in any fashion that you feel is appropriate.
Also, in whatever way you choose to advertise yourself on my site, I do ask that you provide a link to this site on your site as well. Read this FAQ to see why I think you should link to this site.
I have created a Tall site badge and a Wide site badge.
You may download the badges to place on your server. Alternately, you may set the "img" setting to point to my server. They are a little crude now (like my logo on this site); I created them through paint. So, if you would like to just use a text link, that would be fine.
For those who would like it, here is some sample HTML code below to add an image link with the wide badge image pulled from my server that will open in a new window.
